Intermediate Care Facilities

Quest, Inc. Intermediate Care Facilities (ICFs) offer a nurturing home environment for individuals with developmental disabilities who are medically complex.

Quest operates three ICFs, two in the Tampa region and one in Orlando. ICF care is 100% Medicaid funded. There is no ability to serve individuals through other funding sources. ICF long-term Medicaid Waivers are approved through the Agency for Persons with Disabilities (APD). By state statute, APD must approve or deny an application for a long-term waiver within 90 days.

Listed below is the placement process based on an individual’s long-term waiver status.

APD Referral

If a person currently has a long-term waiver and APD is facilitating the referral:

  • Referral goes out to multiple agencies from APD, typically by region
  • Quest reviews referral for appropriate fit for a Level 2 client with no high-behavior needs
  • Quest meets with viable placements for in-person screening
  •  Quest coordinates with family/guardians the client’s transition into their new home
  • Guardians and family members have a choice in placement and may request transfer to different ICF locations

Community Placement with Long-term Waiver

If a person is seeking a transfer from a current ICF, is in the hospital, or their level of care has risen since previous placement:

  • Inquiry is placed into Quest, or garnered through outreach
  • Quest Residential Liaison solicits care plan
  • If bed is open and client is a viable placement, an assessment visit is scheduled
  • If approved for placement, Directors and nurses work to transition client into facility

Community Placement with No Current Long-term Waiver

If a person may qualify as needing long-term 24-hour nursing care, is medically complex and has a developmental disability, but does not currently have a long-term waiver:

  • Inquiry is placed into Quest
  • Quest apprises inquiry contact of APD long-term waiver process and provides APD contact info
  • Quest Residential Liaison keeps in touch monthly on progress of waiver application
  • Quest Residential Liaison solicits care plan
  • If bed is open and client is successfully granted a long-term waiver, an assessment visit is scheduled
  • If approved for placement, site directors and nurses work to transition client into facility
